APCM is structured into three levels, based on chronic condition burden and socioeconomic factors:

Level 1 (G0556): 0–1 chronic conditions

Level 2 (G0557): 2+ chronic conditions

Level 3 (G0558): 2+ chronic conditions and Qualified Medicare Beneficiary status
